Even skin tone and a smooth texture are two important qualities of skin health. Skin tone is determined by the amount of melanin or pigment we have. Skin texture refers to our skin’s surface condition. Ideally, skin should be soft with a smooth, even texture, but it’s not uncommon to have skin that feels coarse and irregular.

Skin tone can become uneven for a number of reasons. Skin blotches, pigmentation, dark patches, and age spots are all conditions where pigment is unevenly distributed, giving our skin a dull and splotchy tone. An uneven skin tone, called hyperpigmentation, is irregular darkening of the skin. The cause: overproduction of melanin, the pigment responsible for skin and hair color. The resulting dark patches and spots are typically perceived as “aged skin” and can make you look more than 12 years older.

Sun exposure: When your skin is repeatedly exposed to UV light, sun damage occurs. Brown spots and uneven skin tone develop as a result of too much melanin being produced to help protect your skin from UV light.

Pollution: Particles and gasses from traffic-related air pollution are able to penetrate the skin and lead to formation of brown spots, particularly on the face.

Hormones: Melasma is hormone-related hyperpigmentation caused by increased hormone stimulation. It’s most commonly experienced by women who are pregnant (which is why it’s also known as the “mask of pregnancy”) or taking contraceptives, but can also be a reaction to cosmetics or medications.

Post-Inflammatory Hyperpigmentation: This is a darkening of the skin as a result of scarring, which can be caused by acne lesions or skin injury.

How to Determine your Skin Type

Before you can learn how to treat uneven skin, you first need to determine your unique skin type. Skincare routines must be tailored around your skin, otherwise you run the risk of worsening your complexion concerns by making your face unduly dry, oily, irritated or blemished. Generally speaking, there are four types of skin:

  1. Dry skin: Dry skin produces less sebum (a sticky substance generally known as oil) than normal skin. As such, the skin lacks the lipids it needs to retain moisture and shield against environmental influences.
  2. Combination skin: As the name suggests, those with this skin type have a combination of dry, oily, and normal areas within the complexion. The skin type will generally vary between the T-zone (forehead, chin, and nose) and the cheeks.
  3. Normal skin: “Normal” is the term used to describe well-balanced skin. The T-zone might be a bit oily, but overall the sebum and moisture are balanced in ratio.
  4. Oily skin: Oily Skin has a heightened sebum production. The face may appear excessively shiny, with pores frequently enlarged and visible.

 

You don’t have to spend a fortune on laser treatments and prescriptions. Here are ways to safely and non-invasively brighten skin for a more even skin tone:

Apply SPF daily. Unprotected sun exposure is the number one culprit behind hyperpigmentation. Applying a sunscreen or moisturizer with a Broad Spectrum SPF of at least 30 is the easiest way to help prevent new spots. Tip: Look for Oleosome technology, which increases SPF performance.

Exfoliate. Treat existing hyperpigmentation by first sloughing off dead, dulling skin cells at the surface. This clears the path for the ingredients in your serums and moisturizers to better penetrate the skin. Tip: Look for a gentle, micro-fine exfoliant you can use daily.

Use the right ingredients. Seek out products that contain Activated Charcoal to help remove pollutants; Oligopeptides, Niacinamide and Zinc Glycinate to help control melanin production; and Retinol to restore skin.

Be patient. There’s no quick fix for an uneven skin tone. Traditionally it takes at least 30 days of vigilant skin care treatment before you see any progress. Successful, 75 percent skin brightening can take up to four months, so be patient.

 

  • Premature ageing can present as brown spots and dots, as well as warts and lumps
  • Superficial pigmentation can be treated with Fraxel laser, Clear + Brilliant, BBL, Chemical Peels, PicoWay Laser, Microneedling, EndyMed FSR and Medical Facials
  • Erbium lasers like Fraxel can also treat deep pigmentation but also address skin texture and most importantly deep wrinkles

Brown spots and skin pigmentation can be due to a variety of causes, and a diagnosis is absolutely essential before treatment. If diagnosed incorrectly, pigmentation can actually WORSEN with treatment. A classic example is melasma – skin pigmentation that is caused by excessive overproduction of melanin (brow pigment) by skin cells. This condition should be treated with creams and peels before laser.

  1. Sun-induced hyperpigmentation

Sun exposure (actinic pigmentation), freckles, and skin ageing

One of the primary functions of melanin production in your skin is to protect you from UV rays from the sun. A “tan” is the prime example of your body’s melanin response to the sun.

When the skin is assaulted by repeated exposure to the sun, you experience sun damage in a variety of ways and one of those is brown spots and hyper-pigmentation resulting from an over-production of melanin. Once the cycle of pigment over-production has started, it is difficult to stop it and usually medical intervention is required.

  1. Hormonal hyperpigmentation (melasma or chloasma) caused by hormone changes

The second most frequent cause of hyper-pigmentation is hormone changes. This may be a result of birth control pills, pregnancy or the various stages of menopause. Hormone shifts trigger the melanin cells to go into over-drive. This can happen even if you are not a sun worshipper. Sun exposure will always make it worse.

  1. Post-inflammatory hyperpigmentation caused by injury

The third most common cause of pigmentation is a direct result of some type of injury to the skin. Typical pigmentation-causing injuries include:

  • Improperly performed laser treatments
  • Acne or Acne Cysts
  • Over-exposure to heat
  • Hair waxing
  • Insect bites or itchy rashes
  • Abrasions or cuts

Inflammation is your body’s response to injury. Often inflammation is not obvious because it is occurring at a deep level. This inflammation triggers melanin production and the result is a concentration of pigment in the affected area. A common result of improperly performed hair laser is a proliferation of dark patches on the legs or face. There are a variety of hair lasers on the market and not every one is designed for every skin type and color.

Most of our treatments are laser and light based procedures. This gives the greatest improvements in skin pigmentation and age spots. The ideal treatment will depend on your diagnosis. Very mild skin pigmentation and brown spots can respond to creams, conversely long standing pigmentation will only improve with lasers.

The 6 methods to improve skin pigmentation include –

Creams and Skin Care – these are prescribed to help maintain your skin after laser treatment for pigmentation. They can also be helpful in reducing pigment due to melasma, and very mild forms of sun damage. We like to recommend combination ingredients that contain Vitamin A, Vitamin C, AHAs, as well a bleaching and pigment correcting agents. Sunscreen forms the basis of your daily skin care regime.

Chemical peels– can be used for mild pigmentation and freckles.

Lasers – this is the mainstay of removing age spots, sun spots and pigmentation. Depending on the depth of pigmentation, we use specific lasers. FRAXEL is a great option for deeper pigment, however we do combine PicoWay, Clear and Brilliant Permea and BBL in most of our laser programs. Clear+Brilliant is an excellent laser to maintain your skin after skin pigmentation has been treated. PicoWay is an excellent option for spot treatments as well.

BBL  Photorejuvenation – an advanced form of Intense Pulsed Light (IPL) technology is one of the most effective treatments for dark spots. BBL is a treatment that works to rejuvenate the skin by encouraging skin cell turnover and new collagen production. On dark spots, this treatment reduces the unnecessary melanin that produces excess pigment.

Medical Facials – Facials such as SilkPeel Dermalinfusion, Hydrafacial MD, DMK Facials or Oxygeneo, or Intraceuticals Oxygen treatments are some non invasive ways to help reduce superficial pigment and to help maintain results post laser treatments.

Microneedling – Can be an additional option for superficial pigmentations or as a maintenance treatment after skin pigmentation has been treated.

Sun exposure is the primary cause of facial and body brown spots, age spots, freckles, moles and pre-cancerous actinic keratosis. Preventing sun-induced hyperpigmentation is a full time process. Hormones, injury and pigmentation from allergic reactions or skin disease such as acne are mostly beyond your control.

Sunscreen, sunblock, hats and sun-protective clothing are your first line guard against not only unsightly brown patches but also sun damage that could lead to skin cancer.

Some types of pigmentation can be improved with creams. Dark pigmentation such as melasma a classical example where we use creams and gentle peels instead of laser to treat this condition. Mild freckles and very early sun induced pigmentation can be improved with a mixture of Vitamin A creams, Vitamin C serums, AHAs and bleaching agents and pigment regulating creams.

In most cases of pigmentation, lasers are needs to markedly improve your skin texture.
